Lineage

Your practice, carried forward.

Sell-side advisory for owners of therapy, psychiatry, ABA and IOP practices with $300k to $5M in revenue. We value the practice, find the right buyer and protect what you built.

Why Lineage

Small behavioral health practices are too small for investment banks and often misunderstood by generalist brokers. Payer mix, clinician retention, licensure and owner dependence drive value, and buyers know it.

Built for owner-clinicians planning an exit, retirement or partnership in the next one to three years.

Questions owners ask

How do I sell my therapy practice?

Start with a confidential valuation of your financials, payer mix and team. Then prepare the practice by reducing owner dependence and cleaning up the books. Finally, approach strategic groups, investors and individual buyers, compare offers and negotiate terms. Plan on six to twelve months.

How do I sell a psychiatry practice?

Use the same path as other behavioral health practices: start with a confidential valuation of financials, payer mix and team, prepare by reducing owner dependence, then approach strategic groups, investors and individual buyers. Lineage advises psychiatry practice owners through that process. Plan on six to twelve months.

How much is my therapy practice worth?

It depends on your earnings, growth, payer mix, clinician retention and how much the practice depends on you. We start with a confidential review and share an indicative range on a call.

Should I sell my practice to private equity?

Private equity is often a fit only for larger groups with growth potential and clean financials. Strategic groups and individual buyers offer different prices, terms and roles for you after closing, so we compare offers on total value, not just the headline price.

What does Lineage charge?

The first estimate is free. A full valuation costs $2,500 to $5,000 and is credited at close. The success fee is 8% to 10% of the sale price under $1M, with a $25,000 minimum, and 5% to 6% from $1M to $5M.

What is the difference between the free estimate and the full valuation?

The free estimate starts with a confidential look at your numbers and ends with an indicative range shared on a call. The full valuation is a deeper review of your financials, payer mix and team, costs $2,500 to $5,000 and is credited at close.

How long does a sale take?

Plan on six to twelve months.

Will my staff find out?

Not until you decide. Buyers sign confidentiality agreements first.

Do you buy practices yourselves?

If an affiliated buyer is ever interested, we disclose it in writing before any information is shared.

What is an earnout when selling a practice?

An earnout is extra payment you may receive after closing if the practice hits agreed targets. It sits alongside other deal terms such as rollover equity, seller notes, holdbacks and employment agreements. Lineage walks clinician owners through these terms in plain language. Read the earnouts and deal terms guide.

How do private equity, strategic and individual buyers differ?

Private equity platforms, strategic groups and individual buyers differ in price, terms, culture and the role they want you to play after closing. Compare offers on total value, not just the headline price. See the buyer comparison guide.

What multiple do mental health practices sell for?

There is no single published multiple that fits every practice. Value depends on earnings, growth, payer mix, clinician retention and how much the practice depends on the owner. Lineage starts with a free estimate and shares an indicative range on a call; a full valuation costs $2,500 to $5,000 and is credited at close.

What do buyers look for in a psychiatry practice?

Buyers care about prescriber capacity, referral stability, payer contracts, telehealth reach and how much revenue depends on the owner. Preparation around staffing, compliance and clean financials matters before you go to market.
